How the Enrollment Process Works
I’ve made enrolling at Ummie’s Daycare simple and straightforward. Here’s what to expect:
Step 1: Complete the Enrollment Form
Start by filling out my enrollment form via the Google Form here or below. It takes just a few minutes and helps me get to know your family. I ask for essential information about your child and any special needs or preferences you’d like me to know about.
Step 2: Submit Your Form
Once you’ve completed the form, simply submit it online—there’s no need to email it or bring it in person.
Step 3: Schedule Your Tour & Orientation
You can now skip the wait and schedule your tour and orientation directly using my Google Calendar: book your visit here. Choose an available time that works best for you, then come see my warm, nurturing space, meet me, and ask any questions you’d like—easy, simple, and fun.
Step 4: Welcome to the Ummie’s Family!
Your child’s spot is secured once the $50 registration fee is paid! I’ll then send you a complete welcome packet with everything you need to get started—from my daily schedule and care policies to tips for a smooth transition. I’m here to support you every step of the way.

Pay Your Registration Fee
Once availability is confirmed, secure your child’s place with a one-time $50.00 registration fee. Payment is processed securely through Stripe.
What Comes Next? (Spoiler: Paperwork. So Much Paperwork.)
Maryland state regulations require a few forms to be completed. Go ahead and grab a coffee ☕ — yes, it’s a lot of forms, but each one is designed to keep your child safe, healthy, and protected. And the best part? You don’t have to figure it out alone — I’m here to walk you through every single page.
You can download the forms below or pick them up. Parents have two weeks after their child’s first day to have the medical provider portions completed — so no stress if your pediatrician can’t squeeze you in right away!
And There’s More… (Yes, Really. But Trust Us, It’s Worth It.)
Here’s something important: all of the documents above & below are also mandatory by the State of Maryland Office of Child Care. Every licensed childcare provider — including Ummie’s Daycare — is required to use them. Instead of downloading them here, you will receive these forms directly from me, and all completed forms must be returned before your child’s first day. And here’s the thing — this isn’t just bureaucracy. It’s how Maryland ensures every child in licensed care is safe, respected, and well cared for. We’re proud to follow these guidelines because they reflect our values.
Please review and sign each form below before your child’s first day:
- Our Family Handbook – The ultimate guide to life at Ummie’s. Think of it as the cheat code for a smooth, happy daycare experience.
- Activity & Transportation Permission Slip – So your little one can join in on all the fun (and actually leave the building with us).
- Parent/Provider Contract – We both sign it, we both mean it. It’s how we build trust from day one.
- Positive Behavior Policy – Because even toddlers need to know the house rules. Kindness, respect, and no biting.
- All About Me Form – The BEST one. Tell us everything — favorite toys, comfort items, quirks, food preferences. This helps us love your child the way THEY want to be loved.
Many of these form are for you to read over, but once all the paperwork is done, the fun begins! 🎉 I promise you, watching your child thrive in a space where they’re truly seen and celebrated makes every form totally worth it.
